We will be holding a registration Saturday this weekend April 27th from 8-11am.
You must have ALL required documents during your chosen registration date and time:
*Completed Blue Affidavit form
* Current Proof of Residency with YOUR name and address stated
*SUHSD required registration forms
*Student Immunization Record
*Complete On Line Registration
* Most recent 6th grade report card
*Returning 8th Graders are only required to complete 2024-2025 online registration UNLESS you have moved since previous school year. If you have moved, you MUST submit Completed Blue Affidavit and Current Proof of Address with your name and Address for your student.
What do you need to Register?
Please fill out OLR Household Form. This form must be filled out in order to create an Online Registration account for your child. Once you have submitted your form, we will email account information and next steps to you within 2-4 weeks.

If Chula Vista Middle is not your school of residence, you will need to apply for an Intra district transfers. Transfers are approved at a district level. School sites do not approve their own transfers. Open Transfer request window is open from January 29th to March 1st. Transfer applications must be submitted via your Infinite Campus Parent Portal.
*ALL transfer requests must initiate at your homeschool.*
Results will be emailed to you by the end of March 2024 directly from our district offices.
